Artis Driving ® - Student Agreement

Student by signing below hereby enters into this agreement with the Driving School for one - Cost $,

service package. Instruction and Third Party Administration (TPA) of the Class E Skills Exam, if selected, will be done by state licensed personnel. See the name field above for registration information on the vehicle used for the TPA Class E Skills Exam (if selected)

Cancellation Policy: 24 hour notice to avoid penalty. A lesson is forfeited if the instructor arrives to the scheduled lesson location and the lesson cannot be completed or is not cancelled within 24 hours of the scheduled time.  A 3% transaction fee applies to any amounts refunded.

For TPA Exams: All applicants are subject to random selection for re-examination at the DMV office prior to the issuance of any license. Failure to pass the random re-exam will require the applicant to pass the driving skills exam prior to license issuance.

My signature below certifies that I have requested that Artis Driving ® provide driving lessons in my vehicle.  I understand that since the driving lessons will be done in my vehicle that all liability during the lessons will be transferred from Artis Driving ® to me.
